Vaccination & Health Records Your Child Needs for School Admission
Most schools ask for a vaccination record at admission — and it protects every child in the classroom. Here is the immunisation and health-document checklist Indian parents should keep ready.

Alongside the birth certificate and address proof, most schools ask for your child's vaccination record at admission. It is not bureaucracy — it protects every child in a shared classroom. Here is what to keep ready.
The immunisation record
Keep your child's immunisation card (the one your paediatrician or the government health centre updates at each visit). Schools typically want to see that routine childhood vaccinations are up to date as per India's Universal Immunisation Programme and your paediatrician's schedule.
Common documents schools request
- Immunisation/vaccination card (original plus a photocopy).
- A general fitness or medical certificate from a registered doctor (some schools only).
- Blood group record.
- Details of any allergies, chronic conditions or regular medication — share these clearly with the school.
- Emergency contact numbers and your child's doctor's contact.
Why schools ask — and what good ones do with it
A responsible school records health information confidentially, keeps a trained nurse or first-aid-trained staff on campus during school hours, and has a clear protocol for contacting parents in a medical situation. When you visit a school, ask where the sick bay is and how medical emergencies are handled — it is part of a school's safety policy.
Keep a simple health folder
Maintain one folder — physical or digital — with the immunisation card, blood group, allergy notes and doctor's contact. You will need it not only at admission but every time your child changes schools, joins a camp, or travels. It takes ten minutes to set up and saves stress later.

Student care at YajurFrequently asked questions
Is a vaccination record mandatory for school admission in India?
Many schools require it at admission to protect all children in shared classrooms. Requirements vary by school and state, so confirm with the specific school's admissions office.
What if my child missed a vaccination?
Speak to your paediatrician about a catch-up schedule. Most schools accept an up-to-date immunisation plan; the key is that routine childhood vaccinations are being kept current.
Should I tell the school about my child's allergies?
Yes — always, and in writing. Schools need to know about allergies, asthma or any condition requiring medication so they can keep your child safe and respond correctly in an emergency.
